4.11.7 Critical Incidents in Foster Placements

This document sets out the procedures to be followed in relation to keeping and maintaining records of placements and critical incidents in placements with foster carers.

1. Introduction

It is important that significant information about placement with carers are clearly charted and recorded.  The Fostering Services Regulations 2002 (30-34) and National Minimum Standards require that a record of each placement with a foster carer is maintained.


2. The Placement Record Form

The placement record form should be accessible at the front of each carer's file under the case file front sheet and should be completed by the supervising social worker who needs to ensure that it is accurate and up-to-date.

The placement record form will record each placement with the foster carer and provide the following detail:

  • Name, age and sex of each child placed
  • Date placement began
  • Date placement terminated
  • Circumstances of termination

It is the responsibility of the Admin Section that a Placement Record Form is placed at the front of all carer's files.

It is the responsibility of the supervising social worker to complete the placement record form.

It is the responsibility of the supervising social worker to complete a green change of circumstances form when there is any change in respect of a placement.

It is the responsibility of Admin to ensure that this information is entered into the database.

The Team Manager must check on a quarterly basis that the information is currently up-to-date.


3. Critical Incident Record Form

The Critical Incident Record Form provides for detail in respect of Standard of Care, Child Protection and Complaints and should be completed when an incident is reported.

It is the responsibility of the supervising social worker to complete a Critical Incident Record Form in respect of any standards of care, child protection and complaints investigation.  The supervising social worker must sign and date the form and pass it to the Team Manager for signature.

The supervising social worker should ensure that this form is held on the carers file in the recording section.

The Team Manager must ensure that the form is completed at the end of every incident relating to Standards of Care, Child Protection and Complaints.

The supervising social worker should also complete the Summary of Critical Incidents Form  and this should be placed at the front of the carers file under the case file front sheet.

The Team Manager will audit compliance on a quarterly basis.

The Team Manager will ensure information is passed to Admin to update the central record on allegations and complaints.

Admin to maintain a central record to bring together data on allegations and on complaints.

The Registered Manager will notify OFSTED and the persons indicated in Schedule 8 Fostering Services Regulations 2002 in the event of any incidents listed.  Any notification made in accordance with this regulation which is given orally will need to be confirmed in writing.
